We’re no longer including nomination statements with fines sent by post

The easiest and quickest way to submit a nomination or court request is on our website. Using our website also means you can submit the form at a time that suits you.

Right now, when you receive an Infringement Notice or Penalty Reminder Notice by post, a nomination statement and court request form is also included. We will stop doing this, from 4 December 2025.

We’re making this change because our data shows nearly 90% of nomination and court request forms are submitted online. Not including them with fines sent by post, means we reduce how much we print and support our department’s commitment to the environment.

You can still call us and ask for a form to be sent to you by post or email. You can also download and print one of the forms below and send the completed form to us by post.

If you send your form by post, it can take time for us to get it. The form needs to be received by us before the due date on your Infringement or Penalty Reminder Notice.

• If your fine has a Victoria Police logo – download the Nomination statement (PDF) and follow the steps in the form.
• If your fine has a Safe Transport Victoria logo – download the STV nomination statement (PDF) and follow the steps in the form.
