      Concession scheme for COVID-19 Fines – form upload

      Please upload your completed COVID-19 fine concession application form and a clear photo/scan of your concession card.

      Completed a driver nomination form?

      If you’ve filled out a form to nominate another driver, enter those details online to ensure the nomination is processed as quickly as possible.

      Once your fine has progressed to a Notice of Final Demand, it is too late to nominate.
